The membership of this Society shall be composed of four classes:
a. Member c. Honorary
b. Associate d. Emeritus

DEFINITION OF A MEMBER:
A “Member” is any physician who holds a degree of Doctor of Medicine, Doctor of Osteopathy; or equivalent medical degree as determined by the Council of the Chicago Laryngological & Otological Society. The “Member” must hold a valid license to practice medicine in Illinois and be certified by a medical specialty board acceptable to the Council to be eligible to become as “member”.
The Council may waive the requirement that a candidate for membership be licensed to practice medicine or be certified by an acceptable board, if such a physician has aided significantly in the progress of medicine as relate to diseases of the ear, nose, and throat, and related structures of the head and neck.
The candidate for membership shall file an application with the Secretary-Treasurer on such form that may be prescribed from time to time by the Council.
The candidate for membership shall be proposed to the Society through the Secretary-Treasurer by two members. An initiation fee of ($100.00) must be made to the Secretary-Treasurer within one month after election to the Society. The Secretary-Treasurer shall then present the candidate to the Council together with a statement of his qualifications for membership. The Council has the sole right to pass on a candidate’s qualifications and the sole right to nominate for membership.
Nomination of the candidate by the Council shall be announced to the Society by the Secretary-Treasurer at the next monthly meeting. If approval of the candidate by the Council has been given during a council meeting, this announcement may be made at the same meeting anytime prior to the adjournment providing a quorum (15 members) is present.
The Secretary-Treasurer shall include in the following month’s program to be mailed not later than one week prior to the date of the meeting, notification that the nomination of the candidate by the Council will be voted on the by the membership. Election must be written ballot in executive session. Five negative votes disqualifies a candidate for membership. In this event, the initiation fee shall be returned if originally received with the application.
An “Associate” member is any person who is engaged or has been engaged in a field which is in the view of the Council allied to Otolaryngology-Head & Neck surgery and is not eligible for any other class of membership in the Society. This candidate can then become eligible for “Associate” membership. Election to “Associate” membership shall be proposed by the Council and approved by the unanimous votes of members present at the regular monthly or Annual Business meeting in April. This candidate’s application must be proposed by two “Members”, approved by the Council, and election must then proceed as for “Members” as outlined in ARTICLE III. “Associate” members will not be eligible to hold office or be members of the Council and have no voting rights. “Associate” members pay $50.00 dues each year.
Election to “Honorary” membership shall be proposed by the Council and approved by unanimous vote of members present at the regular monthly or Annual Business meeting in April. This class of membership shall be limited to guests from other cities or foreign countries who have outstanding reputations for productive research, exemplary teaching, or for extraordinary clinical experience and judgement in the field of otolaryngology. “Honorary” members do not hold office, are not members of the Council, do not vote, and do not pay dues.
The Council may designate “Emeritus” members who shall be relieved of further payment of dues. See BY LAWS-ARTICLE X, 3.
